Join us on June 4th from 5-7pm at our Kingman Family Practice Clinic location.
Bring your sports physical form, $20 and if under age 16, please bring a parent or guardian.
No appointment necessary! Please have the parent/student part of the form filled out prior to coming to this clinic.
This is open to all area student athletes.
